The Alzheimer Society of Ontario (ASO) program, First Link Care Navigation, connects people living with dementia and their care partners to information, supports, and services. CCBR is conducting an evaluation of the First Link program involving over 30 Alzheimer Society sites across Ontario. CCBR will develop a comprehensive two-year Evaluation Framework for ASO and subsequently help implement this multi-site, multi-method evaluation. This project involves managers, staff, people who live with dementia, their care partners, the external community, and primary care providers.
